Position Overview

The Care Medical Practice model is designed for deep engagement with high-risk populations, emphasizing event-driven care management. We leverage care pathways and evidence-based guidelines tailored specifically to underserved populations.We are seeking a versatile and compassionate Family Nurse Practitioner (FNP) who specializes in Women s Health but possesses the clinical breadth to address primary care concerns. The ideal candidate will serve as a clinical anchor for our members, providing expert gynecological and maternal care while simultaneously managing chronic conditions and acute primary care needs as they arise. You will work as part of an interdisciplinary team to ensure high-quality outcomes for patients and families navigating complex medical and socioeconomic landscapes.

Key Responsibilities

Women s Health & Maternal Care

  • Conduct comprehensive well-woman exams, preventive screenings, and diagnostic female-related evaluations.
  • Provide full-spectrum contraceptive counseling and services, including the insertion of long-acting reversible contraception (IUDs and implants).
  • Manage patients through the perinatal period, including pregnancy confirmation, dating, fetal monitoring, and ultrasound performance.
  • Diagnose and treat sexually transmitted infections (STIs) and provide intimacy and sexual health counseling.
  • Offer menopause education, infertility counseling, and support for unexpected pregnancies.
  • Perform specialized procedures such as Pap smears and endometrial biopsies.

Primary Care & Chronic Disease Management

  • Address and manage primary care concerns that arise during women s health visits, ensuring holistic patient management.
  • Perform comprehensive history and physical exams for risk adjustment and regulatory assessments.
  • Order and interpret pertinent diagnostic laboratory and radiology testing judiciously.
  • Create and implement individualized medical plans of care for chronic conditions (e.g., hypertension, diabetes, asthma) common within our target population.
  • Provide post-discharge coordination to reduce hospital readmission rates and emergency room utilization.

Care Coordination & Clinical Operations

  • Conduct face-to-face visits in both clinic and in-home settings, as well as synchronous video Tele-Health communications.
  • Collaborate with OBGYNs and other specialists to provide seamless prenatal and specialty care.
  • Screen for domestic violence, substance use disorders, depression, anxiety, and other social determinants of health.
  • Maintain timely and accurate documentation within the Electronic Medical Record (EMR) system.
  • Engage in collaborative practice under the medical direction of a licensed physician in accordance with New York State laws.

Required Qualifications

Education & Licensure

  • Current registration and licensure as a Registered Professional Nurse (RN) in New York State.
  • Certified and registered as a Family Nurse Practitioner (FNP) by the New York State Education Department (NYSED).
  • National certification as an NP by an organization acceptable to NYSED (AANP or ANCC).
  • Completion of pharmacology coursework and possession of a valid DEA registration.

Experience & Skills

  • 3 5 years of experience as a Nurse Practitioner, with a strong background in both Women s Health and Primary Care.
  • Proven experience working with underserved populations facing socioeconomic barriers to healthcare.
  • Proficiency in phlebotomy, immunizations, and basic office-based procedures.
  • Fluency in a language in addition to English is highly preferred.
  • Strong clinical judgment and the ability to identify when a patient s condition requires physician oversight or specialty referral.

Overview

Duties & Responsibilities

  • Deliver comprehensive and follow-up care through in-home and virtual visits, including AWVs, Comprehensive Assessments, Follow-up visits, and TOCs (Transitions of Care) in accordance with Medicare, Medicaid, and commercial plan requirements.
  • Practice value-based, home-centered care focused on quality, cost reduction, population health outcomes, and patient satisfaction.
  • Provide ongoing management of acute and chronic conditions, collaborating with multidisciplinary teams to develop and implement individualized care plans.
  • Participate in daily huddles and interdisciplinary team meetings to enhance patient care coordination.
  • Prescribe medications, therapies, and lifestyle interventions within scope of practice.
  • Communicate clearly and compassionately with Family Members, supporting prevention, chronic disease management, and health education.
  • Provide on-call backup support to the nurse line in licensed states, requiring additional state licensure for cross-coverage.

Qualifications

  • Education/Experience: Master s degree in nursing or related science field required
  • Certification/Licensure: Active and unrestricted NP/PA license required in the state of hire.
  • Clinical: 2+ years of direct patient care as a Nurse Practitioner strongly preferred. Community-based care experience preferred. Experience with Medicare/Medicaid is preferred
  • 2+ years of clinical experience treating complex chronic conditions and high-needs patients, Medicare and LTSS experience preferred
  • Excellent verbal, written, presentation, and group facilitation skills
  • Must have strong critical analytic, thinking and communication skills
  • Computer Knowledge: Excellent computer skills required particularly Microsoft applications including Word, Excel, PowerPoint, and Outlook. Willingness to explore and learn new or unfamiliar applications that may benefit the clinical team

Overview

Facility is in search of Part Time providers for Locum Tenens to fill in scheduling gaps as needed.


Open Dates of Need, Regular hours, night call, and weekend call:

07/07-07/13


Additional gap dates to pick up as needed.

No regular hours on the weekend.


Shifts available: Clinic hours (Tues-Fri + Mon) 8-430 and take call during the day and at night (call back is very rare at this location)

Board Certified or truly Board Eligible

Certifications: ACLS and BLS

Workload: 1-2 hours depending on the inpatient load

Other Role Notes: EPIC

Support Staff: 3 Clinic RN's, 5 MA's, 3 NP's, 3 Front Office, a clinic pharmacist, and 1 referral coordinator.

Additional Practice Details:


We only see adult patients, no pediatrics.

Typically more Hem than Onc patients but this does fluctuate from month to month

Board Eligible allowed, ABIM would be in Hem, Onc, Internal medicine typically

Volume of patients per day will also fluctuate, it could be anywhere from 15-22 patients- f/u's and new patients

Call varies also, they don't get called back into the hospital, usually just phone calls to answer questions or concerns

New consults per week for Hem and Onc patients will be between 15 and 20 depending on the referral volumes

There will be hospital rounding with a midlevel to assist

If an admit is necessary then yes they will be, our patients usually go through the ER and get admitted from there if needed. Not many direct admits from the office.

Yes, we would like them to participate, come to Tumor Board, only present if they need to

Yes, we have a blood bank

BMBX are done by IR

Chemotherapy is administered in our infusion dept by RN's with a team of pharmacists embedded in the center.

Call would be on the weekends as well. It goes from a Tuesday to a Tuesday.
