Position Overview

Community Care Physicians (CCP) is seeking an experienced or highly motivated Nurse Practitioner or Physician Assistant to join Vascular Health Partners, one of the Capital Region's premier vascular surgery practices.

This is an outstanding opportunity to work alongside four board-certified vascular surgeons and an experienced team of advanced practice providers specializing in the diagnosis and treatment of complex arterial and venous disease. Vascular Health Partners is nationally recognized for its expertise in minimally invasive endovascular procedures, vascular surgery, research, and patient education.

Providers practice primarily from our state-of-the-art office and outpatient interventional suite located at Wellness Way in Latham, with scheduled travel to our satellite offices in Saratoga Springs, Queensbury, and Amsterdam to provide outpatient vascular care.

Practice Highlights

  • Four 10-hour workdays (4-10 schedule)
  • 32 hours of scheduled patient care and 8 hours of dedicated administrative time each week
  • Outpatient-focused practice with no overnight hospital rounding requirements
  • State-of-the-art outpatient vascular interventional suite in Latham
  • Accredited on-site noninvasive vascular laboratory
  • Collaborative team of four vascular surgeons and experienced APPs
  • Opportunity to practice cutting-edge vascular medicine utilizing the latest minimally invasive technologies
  • Practice locations throughout New York's Capital Region, including:

Responsibilities

  • Evaluate, diagnose, and manage patients with arterial and venous disease in the outpatient setting
  • Perform comprehensive vascular assessments and develop treatment plans in collaboration with vascular surgeons
  • Manage patients with conditions including:
    • Peripheral arterial disease (PAD)
    • Carotid artery disease
    • Abdominal and peripheral aneurysms
    • Chronic venous insufficiency
    • Varicose veins
    • Deep vein thrombosis (DVT)
    • Peripheral vascular disease
  • Provide pre-operative and post-operative evaluations and follow-up care
  • Review vascular ultrasound and diagnostic testing results
  • Coordinate care with referring primary care physicians and specialists
  • Educate patients regarding vascular disease prevention, treatment options, and lifestyle modifications

Qualifications

Required

  • Current New York State licensure as a Nurse Practitioner or Physician Assistant
  • Graduate of an accredited NP or PA program
  • DEA registration (or ability to obtain)
  • Strong clinical assessment and communication skills
  • Ability to work independently while collaborating closely with physicians

Preferred

  • Previous experience in vascular surgery, surgery, cardiology, interventional radiology, emergency medicine, or critical care
  • New graduates with prior surgical or vascular clinical rotations and a strong interest in vascular medicine are encouraged to apply

Responsibilities

  • Evaluate, diagnose, and manage patients with arterial and venous disease in the outpatient setting
  • Perform comprehensive vascular assessments and develop treatment plans in collaboration with vascular surgeons
  • Manage patients with conditions including:
    • Peripheral arterial disease (PAD)
    • Carotid artery disease
    • Abdominal and peripheral aneurysms
    • Chronic venous insufficiency

    • Deep vein thrombosis (DVT)

  • Provide pre-operative and post-operative evaluations and follow-up care

Qualifications

Required

The Opportunity:

As the Director of Underwriting - Builder Finance, you will be the subject matter expert for LendingOne's Builder Finance platform. In this specialized role, you will underwrite complex builder finance transactions and help establish standards, workflows, credit guidelines and best practices that will allow the division to scale. What You'll Do:

  • Act as the resident Underwriting expert for our Builder Finance division by underwriting complex transactions, credit analysis and lending recommendations.
  • You will evaluate borrower financial strength, project feasibility, budget, construction schedules, collateral, guarantors and market risk.
  • Structure transactions to balance risk while supporting business growth.
  • Review exceptions and recommend appropriate credit solutions for various lending scenarios.
  • Develop, maintain and refine underwriting guidelines that align with scaling the builder finance product.

What You Bring:

  • Bachelor's degree required; Finance, Business, Economics, Real Estate, or related field preferred.
  • 7+ years of underwriting, credit risk, or real estate lending experience within new construction or builder finance loan types.
  • 3+ years of experience within new construction or builder finance, specifically residential single family/town home communities.
  • You have a demonstrated ability to create efficient and scalable operations within the Builder Finance market.
  • Exceptional communication skills paired with an analytical mind – you are able to work effectively with various departments company wide to problem solve and achieve initiatives while understanding what it takes to mitigate risk within underwriting and credit for business purpose lending.

Where You Will Work:  

Our Level II Labor & Delivery/Postpartum unit provides care for patients delivering at 32 weeks gestation and above, with protocols in place to stabilize and transfer those under 32 weeks to a Level III facility. The unit includes 9 labor rooms, 12 postpartum rooms (shared with med-surg patients, though OB nurses care only for OB patients), 3 triage bays, and 2 dedicated OB operating rooms. We average 100 deliveries per month, totaling 1,183 last year. The team consists of 6 in-house OB/GYNs, anesthesia on-call exclusively for OB (typically in-house or nearby), and a neonatal nurse practitioner available 24/7 for high-risk deliveries. Lactation support is available seven days a week through two full-time and two casual consultants, and an educator with flexible hours supports both day and night shifts. Nurses are trained in both labor and postpartum care, and staffing ratios follow AWHONN guidelines: 1:1 for labor and recovery, 1:2 for cervical ripening and high-risk/magnesium postpartum, and 1:3 for postpartum couplets. The charge RN and triage RN are unassigned to serve as resources to the team. Average length of stay is 24–48 hours for vaginal deliveries and 48–72 hours for cesarean sections. What makes our unit special includes OB emergency simulations held at the Simulation Center in Mount Pleasant, an annual community Baby Fair hosted by the OB/NICU/Postpartum team, a dedicated bereavement workgroup, and prenatal lactation visits offered between 28–34 weeks gestation, along with outpatient and video visit options.

Where You Will Work:  

The Med Surg 2, Med Surg/Peds unit specializes in adult female medical-surgical care, excluding patients with highly communicable infectious diseases due to its proximity to the Postpartum and NICU units. It can also accommodate select pediatric cases when appropriate. The unit shares physical space with Postpartum, typically housing six patients on each side with separate care teams, though MS2 can expand to 9–12 beds if Postpartum does not require the extra space. The average census is around six patients, and the unit commonly cares for adult female patients with medical, cardiac, stroke (non-TNK/ICU-level), and gynecological surgical needs. Pediatric patients may include ENT post-procedures, respiratory conditions like RSV, post-op surgicals, bilirubinemia, and feeding or temperature regulation cases. Psych or impulsive patients who may pose a risk to new mothers or infants are not admitted. Nurse-to-patient ratios range from 3–5, with assignments capped at three when pediatric patients are included; NA ratios follow a similar structure. All RNs and CNAs must have BLS within 30 days of hire, with RNs also needing ACLS and NIHSS before completing orientation, EKG within three months, and pediatric cross-training within six months. PALS, ultrasound IV competency, and certifications are considered beneficial but can be obtained post-hire. The average patient stay is 2–3 days. The unit benefits from lower PPE requirements due to infection restrictions and may have lighter assignments compared to other MS units. It is the only inpatient department at AMCK that can care for pediatric patients, offering a unique variety for staff. Additionally, the shared space with Postpartum allows staff to experience the presence of newborns. The unit seeks RNs with prior experience who are flexible and comfortable caring for both adult and pediatric patients. New hires must complete pediatric cross-training within six months if not already experienced.

Registered Nurse (RN)

Job Summary:

Using evidence-based practice, clinical decision making, compassion and skilled dialogue, the registered nurse leads efforts to create the safest patient environment and the best patient experience. The nurse delivers and coordinates patient-centered care across the continuum to support the successful transition of patients to other healthcare delivery systems or their home communities. The registered nurse is responsible for providing and coordinating comprehensive patient care through the appropriate utilization of assessment, diagnosis, planning, implementation and evaluation, in accordance with established standards, policies and procedures.


Position Requirements:

Graduate of an accredited school of nursing with Current IL RN license

BSN preferred, ADN acceptable with agreement for nurses with less than 12 months of experience to enroll in BSN completion program to be completed in four years.  Nurses with more than 12 months experience in direct patient care must possess a BSN or obtain either BSN or Advocate Aurora Health (AAH) recognized nursing certification within one year of the date of hire or becoming eligible to sit for certification, whichever is later.

Current AHA CPR

Ability to work weekends, holidays, and different shifts in order to accommodate staffing

Certain positions may need to take call

Work environment includes probability of exposure to adverse, hazardous, or unpleasant conditions while caring for the sick

Ability to assist in evacuation of patients in the event of a fire or other disaster

Routinely works with potentially infectious specimens and patients

Ability to respond to patients in a timely manner and access all areas of the unit

Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other team member, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
