Job Summary and Qualifications
As a Respiratory Therapist, you’ll play an essential role in guiding patients through both routine care and critical moments. You’ll perform diagnostic and therapeutic procedures while managing invasive and noninvasive ventilation across a variety of care settings. Your role will include:
- Assessing respiratory status by performing focused exams, interpreting breath sounds and airway patency, and determining the need for respiratory interventions.
- Delivering evidence-based therapy by providing oxygen support, aerosolized medications, airway clearance techniques, and other treatments across emergency, critical care, and acute settings.
- Managing ventilation and airways by assisting with artificial airway placement, initiating and adjusting invasive and noninvasive ventilation, monitoring patient responses, and responding quickly to clinical changes or emergencies.
- Supporting patients and the care team by educating patients and families, serving as a clinical resource, partnering with physicians and nurses, and helping stabilize newborns during deliveries when needed.
- Maintaining equipment and safety by setting up, operating, and cleaning respiratory equipment; ensuring readiness and preventive maintenance; transporting ventilated patients safely; and following all infection-prevention standards.

Job Summary:
Assesses, develops, plans, implements, and evaluates patients discharge planning needs to next level of care in collaboration with interdisciplinary team.Job Responsibilities and Requirements:
PRIMARY RESPONSIBILITIES
- Ensures compliance with Condition of Participation (CoP) discharge planning.
- Identifies and escalates gaps/barriers related to inpatient and observation length of stay.
- Assesses patients at high risk for readmission. Identifies root cause for readmission. Develops plan and collaborates with multidisciplinary team regarding discharge planning to next level of care.
- Participates with other members of team regarding opportunities for improvement in standard work.
- May also be responsible for: utilization management process, ED case management process, admission process, and act as a case manager liaison with post acute providers (skilled nursing facilities, rehab, home health, etc.).
- Applies the existing body of evidence-based practice and scientific knowledge in health care to nursing practice, ensuring that nursing care is delivered based on patient’s age-specific needs and clinical needs as described in the department's Scope of Service.
- As an SSM Health nurse, I will demonstrate the professional nursing standards defined in the professional practice model. Uses the ANA Code of Ethics for Nurses to guide his/her response to the current and evolving health and nursing needs of our patients and our patient populations.

EDUCATION

Job Summary and Qualifications
Provide a patient onboarding experience for patients admitting to the in-patient units. Provide support to in-patient units upon discharging. Responsible for assessing, planning, implementing, evaluating and documenting all aspects of patient care. He/she will work closely with all health care providers to facilitate and coordinate efficient, effective, quality health care. The individual must demonstrate knowledge of the principles of growth and development over the appropriate life span; possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age specific needs; and, have the ability to provide care needed by the pertinent patient group(s). He/she will implement and/or comply with all the policies, procedures and standards of the unit, department and the institution.
What you will do in this role:
- Coordination of care with physicians.
- Actively collaborates with other units and program staff, physicians and members of other departments to facilitate provision of optimal patient care.
- Provide patient education while administering medication.
- Practice safe medication administration consistent scanning of patient and medications.
- Scan all medications and patients using eMar.
- Follow facility established guidelines for reconciliation of medications on admission, during transfer and at discharge.
- Assign and conduct appropriate individual and group interventions within the milieu for behavioral and safe management of patients.
- Maintains appropriate boundaries.
- Demonstrate least restrictive interventions appropriately.
- Collaborate with interdisciplinary team in assessing, planning, implementing and evaluating treatment, patient safety and programs.
- Assess patient’s response to treatment and safety interventions and accurately document interventions and patient’s response to intervention.
- Assist patients and families to understand the diagnosis, prescribed treatment, and safe management of symptoms related to specific mental disorders through health teaching.
- Document against the interventions within the Recovery Plan Utilize teaching skills related to psycho educational and medical interventions and patient safety needs with the patients and families.
- Assumes responsibility for renewing license.
- Assumes responsibility for completing all mandatory educational, health screening, CPR, and CPI requirements within designated time frame and for providing written verification of same for filing.
- Transcribes orders accurately and scans orders to pharmacy in a timely fashion. Provides the new patient with an in-depth on-boarding experience.

Live Oak Mental Health and Wellness Center, is a part of Trident Health. We give inpatient stabilization treatment to patients who seek treatment for acute psychiatric symptoms which cannot be addressed on an outpatient basis. Common disorders of patients treated in this program include but are not limited to: major depressive disorder, schizoaffective disorder, schizophrenia, bipolar disorder, adjustment disorders, post-traumatic stress disorder, impulse control disorder and a variety of co-morbid personality disorders. Our patient population is both adult and adolescent. We offer 24 hour availability of a full range of services. We have the capability to institute rapid lifesaving interventions. Our physician availability is 24 hours a day to institute needed modification to the treatment plan. Psychiatrist involved in the direction and management of the treatment program. Round the clock skilled nursing care implementing the treatment plan and providing monitoring and assessment of the patient's condition and response to treatment.
